4. Self-Pay Policy

Elevate Psychology, LLC is a self-pay practice. We do not accept insurance. Upon request, Dr. Diana Medina will provide a receipt (superbill) containing the necessary codes for you to submit to your insurance provider. It is your responsibility to submit receipts and manage any reimbursement directly with your insurance company.

5. Use of Telehealth Services

Telehealth services are offered to clients in compliance with state licensing requirements. By engaging in telehealth, you confirm that you are physically located in a state where Dr. Medina is licensed to practice. You are responsible for ensuring that your technology is suitable for telehealth sessions and for maintaining your privacy during these sessions.

6. Privacy and Confidentiality

Your privacy is important to us. All information shared during therapy sessions is confidential and protected under applicable laws.
